Patient Information

During your first visit, the doctor will review your medical history and acertain of the reason for your visit. Following the consult, the doctor will perform an examination including: range of motion studies, orthopedic and neurological tests, as well as a palpation examination. You will have plenty of opportunities to ask questions and address any concerns that may arise. Based on the results of the examination, a determination will be made on whether x-rays are medically necessary, and if so, what areas we will need to x-ray. The doctor will then explain the results to you and recommend a treatment plan that is appropriate for your condition. At this point we will begin treatment and start the road to a happier and healthier you.

You will need to bring a picture ID and your insurance identification card (if you are using insurance). We accept cash, personal checks, Visa, MasterCard and Debit card as forms of payment.
